It’s the woman’s own damn fault if she goes to an unqualified “back alley surgeon”. The situation you’ve constructed is basically a woman holding her own body hostage to convince someone else to do what she wants. She needs help and no amount of plastic surgery is going to ever possibly help her.

I’d argue that she does need help, she’s trying to become something she isn’t, and in the process, quite possibly seriously hurting herself. It sounds like she has a mental illness, and no amount of surgery is going to fix that problem, it’s like chopping down a tree trying to find the gold inside the tree, no matter how hard you try, that’s just not going to work.
